Name origin: Collagen derives from the Greek kolla (glue) and -gen (producing) — a reference to the fact that boiling collagen-rich tissue yields glue, which is precisely what gelatin is. It is the most abundant protein in the mammalian body, characterised by a distinctive triple-helical structure built on repeating glycine-X-Y motifs, with proline and hydroxyproline dominating the X and Y positions. Evidence for Collagen Applications

The eggshell membrane evidence in knee osteoarthritis has been meta-analysed, and the result is positive but modest — both halves of which deserve equal weight. A systematic review and meta-analysis searching PubMed, Scopus, Web of Science, and Cochrane through July 2024 identified seven randomized controlled trials meeting inclusion criteria, with five entering the meta-analysis; results indicated that eggshell membrane significantly reduced pain and improved functionality, with a total WOMAC score effect size of −0.34 (95% CI −0.56 to −0.13, p<0.001) and a pain subscale SMD of −0.23 (95% CI −0.42 to −0.04, p<0.02) (Nutrients 2024). By convention an SMD of 0.23 is a small effect. It is real, it is statistically robust, and it is small. Claim strength: Moderate.

The underlying trials are reasonable. A randomised clinical trial assigned three groups — two different doses of eggshell membrane supplement versus placebo — with knee pain by visual analogue scale as the primary outcome and knee functional ability, quadriceps strength (isometric and isokinetic), and sleep quality as secondary outcomes; all groups showed a significant decrease in pain perception, with maximum values in the high-dose group, and the high-dose group showed significant increases in strength on both isokinetic and isometric testing (Cánovas 2022 Nutrients). Note the honest detail: all groups improved, including placebo. Dose-dependence in the active arms is the meaningful signal. Claim strength: Moderate.

The biomarker evidence is genuinely mixed and should not be selectively reported. Some work has demonstrated a chondroprotective signal via reduced cartilage degradation biomarker CTX-II. But a separate randomized double-blind placebo-controlled trial in early-stage knee OA, designed specifically to test whether inflammatory and cartilage markers would fall, found no significant differences in five of six markers (hs-CRP, ESR, TNF-α, IL-1β, MMP-2, urinary CTX-II), with only COMP differing. Why eggshell membrane rather than collagen peptides? The proposition is that it delivers a whole matrix — collagen types I, V and X alongside naturally occurring glycosaminoglycans including hyaluronic acid, chondroitin sulfate and dermatan sulfate, plus elastin and desmosine — rather than a hydrolysed fragment of a single protein. Preclinical work has reported inhibition of 5-lipoxygenase and prostaglandin E2, suppression of the ECM-degrading enzymes MMP-2 and MMP-9, and upregulation of chondrogenic genes (Sox9, aggrecan, Col-2). Whether the matrix proposition is genuinely superior to peptides has not been tested head to head, and honesty requires saying so. Claim strength: Emerging.

A general caution about the collagen category that applies here and is worth stating plainly, because the market almost never does: ingested collagen — in any form — is digested to amino acids and short peptides, and there is no mechanism by which dietary collagen is transported intact to reconstitute collagen at a target tissue.
